Fort House Surgery Wellbeing Advisor Team

Fort House Surgery are pleased to introduce our new Wellbeing Advisor team.

Suzie and Sadie will be seeing patients from April 2026, working alongside the wider surgery team to provide wellbeing advice and support.

They offer support across a range of areas including lifestyle medicine, weight loss, smoking and alcohol cessation, low mood, social isolation, carer support, and social concerns such as debt and housing issues. Both have completed professional training in these areas and can also advise on and refer to specialist services, including exercise on prescription for rehabilitation.

Sadie and Suzie can also refer you to social services and local community organisations to support increasing care needs.

Appointments will be available both face-to-face and over the phone. They will also be setting up regular support groups for specific patient groups, such as carers – so keep an eye out for updates.
