Phlebotomy Service

Fort House Surgery has an in-house phlebotomy service. Appointments can be booked via Reception for the following blood tests:

Blood requests from ‘local NHS hospitals’ (ASPH, Surrey)

  • Patients to either bring in their request form, or it can be printed from ICE in the practice.
  • No additional requests can be added at the request of the patient that are not already on the form.
  • If a patient needs additional bloods because Fort House have requested them, these can be done at the same time.
  • These results will then be sent through to the local hospital.
  • Fort House is not responsible for actioning these results.
  • If the patient requires the results, they should direct this query to the speciality in the hospital who requested the bloods, as the result will go directly back to them.

Blood requests from other NHS hospitals/NHS Specialities or NHS Consultants

  • The request form must be brought by the patient to enable us to do the blood test.
  • If the patient does not bring the blood form, we will be unable to do the procedure.
  • We are unable to request these blood tests within the surgery, and we are not able to use a discharge summary or consultant’s letter instead of a request form.
  • If the patient requires the results, they should direct this query to the speciality in the hospital who requested the bloods, as the result will go directly back to them.

Blood requests from Private Providers (following guidance from the BMA)

  • We are unable to do private blood tests with immediate effect.
  • These bloods do not form part of our GMS contract and we are unable to use NHS facilities to do private work.
  • As such, with immediate effect, Fort House is no longer able to do any private blood tests. This applies even if these have been done in the past.
