New patient registrations

Our list will be closed to enable us to register all new patients who currently wish to move to the surgery.
We aim to re-open our list on 19th January but will put a message here
We are delighted that our planning application to increase our patient list size has been approved, meaning we can reopen our patient list. We will continue to do this in a controlled and safe manner, with limited new registrations allowed.
We open our patient list on a Wednesday morning at 9:00am
Once capacity has been reached, we will turn off the form and no more new patients can be accepted for that week.
**Please note the new time of 9:00am for the forms to ‘go live’**

Registering with the surgery
To register with the surgery:
- Complete an online registration form to register with the surgery
- if you are unable to do an on-line registration form and don’t have access to the internet, please visit our reception between 1.30pm and 6pm and collect a paper registration form.
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
PLEASE NOTE – It can take up to 2 weeks for your registration to be processed. You will still be registered at your previous practice until you receive a text message from us to confirm your registration has been processed.
Named GP
All patients at Fort House Surgery have a named, accountable doctor who is responsible for coordinating their care.
Your named doctor will be allocated to you by the practice.
Whilst we will try and ensure you see the same GP, there will be occasions when it won’t be possible (for example you need to speak or see a GP on the day and your GP isn’t working; or they are on holiday when you want an appointment).
